Part of the biceps muscle runs through the bicipital groove at the head of the arm bone (humerus) in much the same way as a rope runs through a pulley. With overuse, such as after pitching a full nine-innings, the tendon can become inflamed and begin to ache and swell.

Bicipital tendonitis is prevalent among athletes between the ages of 18 and 35 who are active in contact sports that require throwing or stress the shoulders like swimming, gymnastics and martial arts.

There may also be damage to other ligaments or tendons as well, so it is important to take a comprehensive view of all of the soft-tissue systems of the shoulder and assess shoulder posture and symmetry. The primary symptom is pain at the front of the shoulder accompanied by weakness and tenderness. The patient may also sense an impingement when throwing which is called Thrower’s Shoulder. The condition will worsen if it is ignored or not treated and can lead to a torn tendon.
